Support and Services at Home (SASH) is hosting the following events in the month of September! SASH is a state wide program free to older adults with Medicare

WRAP (Wellness Recovery Action Plan) Workshops hosted by Vermont Blueprint for Healthcare & SASH
We all have ups and downs in life. Through the process of building an individualized WRAP (Wellness Recovery Action Plan) we explore what keeps us well, what triggers us to feel poorly and be prepared in case life throws us a curve-ball.
WRAP was designed by Mary Ellen Copeland in the late 1990's in Bradford, VT. It is now offered all over the world!
We will be offering Free WRAP Workshops Every Tuesday for 8 weeks beginning August 30th at 1pm – 4pm.

Stay Steady Vermont
A Falls Prevention Awareness Program to be held at
87 Kelley's Field Rd, Hinesburg VT
DATE/Time: September 27th, 2016 10am-12pm

Do you want to stay active and independent? The biggest challenge you face is falling. One in three adults age 65 and older report a fall each year, but it doesn’t have to be YOU! The Vermont American Physical Therapy Association and State fall prevention coalition wants to help you reduce your risk for falling!
During September, National Falls Awareness Month, we will present "Stay Steady Vermont" events, free balance screening for adults over 65 throughout the state. Each event will offer an informational session followed by a brief balance screening by volunteer licensed Physical Therapists. After screening, people receive recommendations that can help reduce risk of falling, based on individual results.

Please wear comfortable clothes and shoes. Contact with questions. Informational session will be held first and then screenings will start. You are welcome to attend for only the screening

RSVP and the Volunteer Center of the United Way (Informational Presentation)
Where: 87 Kelley's Field Rd. in the Community Room
When: Thursday, September 8th at 10am
Our presenter will talk about the RSVP program, a national volunteer management service which offers individuals of all ages the opportunity to share their experiences, skills and time by volunteering in their community.

Supports and Services at Home (SASH) works to support older adults with:
• Preventative health care and services coordination
• Self-management education and coaching, particularly relating to chronic health conditions such as diabetes and arthritis
• Transitions support after a hospital, nursing home or short-term rehab facility stay
