Pathways Vermont is excited to launch the Community Learning Series - a series of monthly discussions to share what we are learning through the course of our work with the community! We are kicking off the series with a spotlight on our oldest program - Housing First, an evidence-based practice that supports communities to end homelessness. Throught the Housing First model individuals are provided with permanent, independent housing, first, which is followed up with wraparound psychosocial support to address other issues such as employment, mental health and substance use.
Pathways Vermont has been providing Housing First services to Vermonters in need since 2010. In that time we have successfully ended the cycle of chronic homelessness for over 160 individuals and supported an additional 90 individuals successfully make the transition from long-term institutionalization to community living. What's more, the implementation of the Housing First model in Vermont has prevented millions of dollars in psychiatric hospitalization, ER, incarceration and emergency housing costs.
