Advanced Care Planning and Chaplaincy Services during Hospice
Hospice services are more than just for the individual who is in the final stages of an incurable disease. The VNH hospice team also includes Social Workers and Chaplains that help in two very distinct ways.
Advanced care planning can be one of the most stressful things to worry about at any stage of life. Our Social Workers will walk you through the most common advanced care planning documents, what the purpose of those documents are and what steps need to be taken to make them an "active" document.
Processing a loved one's passing is something that we all need support with. Our Chaplains are available to support not only the person receiving hospice, but the entire care team of family and friends for the individual. Our Chaplains will discuss what services are available during hospice care, but how they continue to support the family and friends after their loved one has passed away.
