Our small-group workshops are led by peer facilitators, trained and experienced in delivering this program.
Are you ready to lower your A1C? Our completers did, more than half of our completers reported a significant decrease in A1C after participating in the self-management program. All participants set sustainable lifestyle change goals each week, and reported increased activity levels, and feeling more confident about making healthy food choices.
Here are a few of the ways the self-management tools covered in the workshop can support you:
• Solve problems and create plans for action
• Make healthy eating choices
• Get more physical activity into your daily life
• Better monitor your blood sugar levels
• Prepare for how to safely monitor and manage your diabetes during times of illness
• Find positive ways to manage your stress
• Be an active partner in working with your health care team
