The faculty and students at the Colchester campus of Albany College of Pharmacy and Health Sciences would like to offer you a free vaccination, if you are eligible, against invasive pneumococcal disease. The vaccinations will take place at Armory Lane Senior Housing in Vergennes on Thursday, September 4 at 10:00 AM. If you are interested in receiving a vaccination, please call Diana Rule, (SASH Coordinator) at 870-7182, or Kelly Schulze, (Project Coordinator) at 802-735-2631. At the clinic, you must provide the following: The completed "Physicians Approval" form, and your insurance ID (Medicare Part B or Blue Cross/Blue Shield Anthem). We will mail the approval form to you in advance so you can give it to your physician to be filled out if you wish.

You may also choose to receive a free review of your medications as part of the clinic.
