The Visiting Nurse and Hospice for VT & NH, or VNH for short, offers home health and hospice care to citizens in over 100 towns across Vermont and New Hampshire. VNH provides care for the most vulnerable individuals, including the frail, elderly, and disabled, at-risk families, people with terminal illnesses, children with chronic medical needs, and the uninsured and underinsured. VNH's mission is to provide exceptional home health and hospice services that enhance the lives of those we serve.

While VNH Home Health and Hospice programs are our most well-known services, VNH also offers Long Term Care services and Maternal Child Health services. At VNH, we are committed to supporting individuals from before birth to end-of-life care, and everything in between.
