The Jericho Town Clerk Office will be open from 9am to noon on Sat January 21 to provide notary services for nurses that are needing to complete the background check form related to the Vermont license renewal through the Office of Professional Regulation (OPR). You can fill out the form ahead of time but you need to sign the form in front of a notary.

There will also be tea and coffee in the conference room if you want to sit around and chat with other nurses. I have heard from one nurse who was confused because she had not received anything from OPR about the background check. She learned that she had already completed the background check when she applied for a "compact" license (travelling nurse who holds licenses in multiple States). We also learned together how to download the background check form from the OPR site, so I will have blank forms here if you don't have one. (Thank you Sandra)

I have also heard from other notaries who want to help out. So I think we will be able to accommodate any nurse that shows up (even from outside of Jericho). Jericho Town Hall is located at 67 Vermont Route 15
