FREE HELP to create a written healthcare plan (Advance Directive). Anyone over age 18 should have one - it's how people care for one another.
Sessions offered every Wednesday at 10:00 am or 6:00 pm.

Who do I give the completed form to?
How do I talk with my family about my healthcare wishes if I am unable to speak for myself?
