The Community Health Team of Gifford is offering a FREE healthy lifestyle and weight loss workshop to support you in learning how to eat healthier, develop exercise into your daily routine, meet others who are looking to better their health and wellness, lose weight and prevent type 2 diabetes.

This program is open to anyone who is ready to make a change. Weight loss is hard and we are here to help. A trained lifestyle coach will provide weekly weigh-ins and 24 sessions of lifestyle change information.
Small changes = BIG success.

Please consider this program if you:
-Have been told by your doctor you have PRE Diabetes (Hemoglobin A1C of 5.7%-6.4%)
-Are overweight (BMI over 25)
-Don't get enough exercise
-Have high blood pressure
-Have abnormal cholesterol
-Have a parent, brother, or sister with diabetes

Come for support and knowledge! This program is ongoing through the year.

ROCHESTER:
Beginning September 11, 2018
Tuesday evenings, 5:30pm-6:30pm

RANDOLPH:
Mondays, beginning October 1st 6pm-7pm
Saturdays, beginning October 6th 10am-11am
Gifford Conference Center, 44 S. Main Street, Randolph 05060
