I am proud to announce that there will be a vaccination clinic this Friday, June 4, from 1:00 PM - 4:00 PM at the St. Johnsbury School. We have 102 Pfizer doses that are targeted for our 12 to 15-year-old students of St. Johnsbury School while also open to the general public. To reserve a slot for you or your child, please register in advance. For people 12-17 years old parental consent will be needed prior to receiving the vaccine; consent can be given in three ways:
Make an appointment through the state's registration website (https://vermont.force.com/events/s/selfregistration)
OR
A parent or guardian can accompany the minor to the appointment and can provide consent in person.
OR
If not accompanied by a parent or guardian and not registered in advance, come with a completed Immunization Clinic Consent Form (https://www.healthvermont.gov/sites/default/files/documents/pdf/Vermont-Dept-Health-Walk-In-[...]pdf) and completed Prevaccination Checklist for COVID-19 Vaccine (https://www.cdc.gov/vaccines/covid-19/downloads/pre-vaccination-screening-form.pdf)

Those who are vaccinated at this event will be scheduled on-site for their second dose on Friday, June 25.
