The Rutland VA Clinic is offering a free Advance Care Planning workshop on Wednesday, August 28th, 2-3pm. Veterans and their loved ones are welcome to attend.
We will discuss how to think about, talk about, and then document your preferences for future medical care and medical decision-making. Participants learn from each other as they share their own experiences, values and priorities. Advance directive forms (VA and Vermont advance directives) will be available.
We will discuss:
• The benefits of advance care planning
• Considering values and preferences for treatment at the end of life, during a mental health crisis or if you become permanently unconsciousness
• Choosing the right person to be your healthcare agent
• Talking about your wishes with loved ones and medical providers
• How to complete a medical advance directive
• The difference between an advance directive and code status/DNR
The VA clinic is located at 232 West Street in Rutland.
