On Tuesday, May 8th from 6:30-8pm at the Montpelier Senior Activity Center (58 Barre Road) Michelle Acciavatti, an End of Life Specialist, will lead a workshop about what it means to be an effective health care agent.
Many people have heard that appointing a health care agent is an important part of Advance Care Planning, but what skills does a health care agent need to have? How can you and your health care agent prepare together to make sure they understand your wishes and values? How do you know if you are the right person to be a health care agent?
In a free workshop open to all, we'll answer these questions and more. Come prepared to engage in activities and discussion, and, if you can, bring the person(s) you are thinking of appointing to be your health care agent. It should be a fun and informative evening.
