Prediabetes is when your glucose (sugar) levels are higher than normal. This puts you at risk for developing serious health conditions including type II diabetes, stroke and heart disease.

Join our Diabetes Prevention Program! Developed by the CDC, one of our trained facilitators will educate you about physical exercise goals, healthy eating, stress, and behavior modification. These small changes learned may lead to the reversal of prediabetes. This class is weekly for 16 weeks, followed by twice a month, and finishing with once a month for a total of 22 sessions. This class is brought to you, FREE OF CHARGE, through the Community Health Team of Gifford Health Care and Vermont Blueprint for Health.
