Chelsea Public Library is sponsoring an evening discussion on ADVANCED CARE PLANNING
at the Chelsea Public Library on Thursday, February 16th at 7 PM, lead by Maria Koehler from the Honoring Care Decision Program at Dartmouth Hitchcock
Advance care planning is an important process to help individuals plan for a time when they are unable to make health care decisions for themselves.
It includes:
• Picking someone willing and able to speak on your behalf
• Reflecting on your past experiences with serious illness
• Reviewing your values and what it means to “live well”
• Considering what you would want in the face of expected or unexpected serious health situations
• Having this conversation with your family and care providers
• Documenting this in an advance directive.
Advance directives are tools for clarifying your values and wishes for care in serious health conditions, which will support and guide your family in making decisions for you when needed.
