To qualify for credit, NEW CUSTOMER must reside in a currently serviceable address; must not have been a Burlington Telecom residential customer within the past sixty (60) days of the referral; must order residential service on or before 03/31/10; must maintain two or more base services (Internet, Cable TV, and/or Telephone) with Burlington Telecom for at least sixty (60) consecutive days following installation. If the service is cancelled or if downgraded below two base services in the first 60 days then the full installation charge ($60) will be applied.

*TERMS and CONDITIONS - BT-FPF REFERRAL PROGRAM
This is a promotional program with a start date of January 1, 2009 and an end date of March 31, 2010. Burlington Telecom ("BT" or "Burlington Telecom") reserves the right to end or modify the promotion terms with thirty (30) days notice. BT is not responsible for non-serviceable addresses.

"QUALIFYING NEW RESIDENTIAL CUSTOMERS" (REFERRAL)
REFERRAL must reside in a currently serviceable address; must not have been a Burlington Telecom residential customer within the past sixty (60) days of the referral; must order residential service prior to end date; must maintain two or more base services (Internet, Cable TV, and/or Telephone) with Burlington Telecom for at least sixty (60) consecutive days following installation. The REFERRAL must also maintain an account in good standing.

The actual install date may be later than March 31, 2010 but an order for service must be placed with a customer service representative or through Burlington Telecom's online application prior to 4:30 p.m. on the end date. REFERRAL can use only one REFFERER (e.g., Front Porch Forum) to obtain credit. REFERRAL must mention the REFERRER at the time the order is placed. All orders placed without providing the name of the REFERRER are automatically determined ineligible.

CREDITS and PAYMENTS
The REFERRAL will receive a $25.00 credit applied to their residential BT invoice after the REFERRAL has satisfied the terms and conditions of the offer including maintaining a service and an account in good standing with Burlington Telecom for sixty (60) days.

Employees of Burlington Telecom and their immediate families, as well as all others with whom employees are domiciled, are not eligible to participate in the Program or receive any credits, discounts or other promotional offers under this Program, for referring a customer to Burlington Telecom, for being referred as a customer, or otherwise.
